Community Living and Supports
Community Living and Support is an individualized service that enables the person to live successfully in his/her own home, the home of his/her family or natural supports and be an active member of his/her community. Community Living and Support enables the person to learn new skills, practice and/or improve existing skills.

Community Networking
Community Networking services provide individualized day activities that support the individual’s definition of a meaningful day in an integrated community setting, with persons who are not disabled.
Community Networking services enable the individual to increase or maintain their capacity for independence and develop social roles valued by non-disabled members of the community.

Respite
Respite services provide periodic or scheduled support and relief to the primary caregiver(s) from the responsibility and stress of caring for the individual.

Supported Employment
Supported Employment provides assistance, based on individual circumstances and needs, to explore, seek, choose, acquire, maintain, increase and/or advance in competitive integrated employment.

Supported Living
Supported Living enables people with disabilities the opportunity to live in their own homes. A person who experiences a disability partners with an organization, family, friends and community resources to experience life and become more independent.

Residential Supports (AFL)
Residential Supports provides individualized services and supports to enable a person to live successfully in an Alternate Family Living (AFL) setting of their choice and be an active participant in his/her community. The site must be the primary residence of the AFL Provider (includes couples and single persons).

1915(i)
North Carolina’s 1915(i) services, offered through Medicaid, provide community-based supports that help individuals with intellectual or developmental disabilities (I/DD) live successfully at home and in their communities. To receive 1915(i), a person must have Medicaid, and eligibility or enrollment questions are handled by MCO serving their county. These services help individuals build daily living skills, participate in their communities, access employment opportunities, receive respite support, and, when appropriate, transition into more independent living. Available supports vary by age: children under 16 and students up to age 22 can receive weekly Community Living and Supports hours and annual respite, with older teens and adults also eligible for Supported Employment if they qualify. Adults living at home may receive some services from household members, while minors must receive services from staff outside the home. Overall, 1915(i) provides flexible, individualized assistance designed to promote independence, inclusion, and long-term stability in community settings.

Long Term Community Supports
Long-Term Community Supports provides consistent, day-to-day assistance that helps build and maintain meaningful lives in the community over time. This service is designed for individuals who need ongoing.

Day Supports
Day Supports emphasizes inclusion and independence with a focus on enabling the individual to attain or maintain his/her maximum self-sufficiency, increase self-determination and enhance the person’s opportunity to have a meaningful day. Day Supports is provided in a state licensed facility, where the individuals can receive Individual or Group services based on their specific needs (medical and/or behavioral).
